HIV POSITIVE CLIENT CARE PROGRAM

Just found out you have HIV? Here is how we can help:
  • Connect you to the Infectious Disease Clinic and Public Health
  • Transport and accompany you to appointments and give you appointment reminders
  • Meet you at a location of your choosing to discuss your health concerns
  • Alleviate your fears and educate you on what comes next
  • Advocate for you
  • Visit you in hospital and at home
  • Refer you to other agencies or medical professionals if needed
  • Complete confidentiality
​The Client Care program works directly with people living with HIV/AIDS. It is built upon the understanding that the capacity to manage and maintain health while living with HIV/AIDS is determined, in part, by factors such as income level, housing, access to care, access to health services, access to quality health care, and individual coping skills.  

Our Client Care Coordinator works in conjunction with individuals to determine personal needs and together design supportive plans including: practical needs (food, clothing), health services (transportation, education, medication, and nutrition), support (counseling, alternative treatments), help with housing and referrals to other agencies.

Our Client Care Coordinator provides support on a one to one basis. Our coordinator makes home visits, hospital visits, plans social activities with client and meets the client where they are at. If clients wish to disclose their positive status to individuals, our coordinator aids in facilitating the conversation while debunking myths and enhancing education around HIV/AIDS.

Through the Jerome Nagel and Ric Ranger Wellness Fund, our Client Care Coordinator can provide financial support including help with medication, transportation, infant formula, and psychological counselling.

Below is a client video story. This video tells the story of Charles Redwood's recovery from sexual abuse, addictions, incarceration and HIV to eventual sobriety and dealing with shame. It also details how we managed to help him on his journey to recovery. It is an inspiring story of courage, faith and dealing with a life's journey that was anything but easy. Charles tells his story simply and in a straightforward manner. It is an inspiring message to us all.
